For SIDA/AOA badges call 541-504-3088 or email [email protected]. When submitting a completed application, the applicant will need to present valid identification and manager/supervisor’s signature of approval.
When arriving for an appointment, please park in the paid parking lot (do not park in the Rental Car Parking lot or the Vendor lot) and bring your ticket stub in so that we can validate it.
